"School Insurance" for Students
Each school year parents and guardians have the ability to purchase “school insurance” for their son or daughter. Our school insurance carrier, LEFEBVRE INSURANCE, has converted to an electronic method of providing you with an insurance form as well as school insurance information.
If you would like to purchase this optional insurance policy for your son or daughter, download and print out the form, complete the ENROLLMENT portion of the form, and have your son or daughter return the form to the ADMINISTRATION OFFICE at Diman with the corresponding payment in a sealed envelope.
